Human Error and Smart Supervision in Engineering

luglio 16, 2025
Human error, fragility and smart supervision — have you ever wondered how an advanced system can follow a clearly self-destructive command?

On July 10, 2025, the preliminary report on Air India Flight 171 revealed that just seconds after takeoff, both engines were manually shut down.
The aircraft crashed. 241 passengers and 19 people on the ground died. One person survived.
By chance? By miracle? Or by some geometric fate?

The Paradox of the Perfect System

As a mechanical design engineer — working with gears, transmissions, and moving parts — I know no component is infallible.

That’s why we integrate:
  1. Safety clutches
  2. Torque limiters
  3. Fail-safe systems
  4. Redundant controls
Not out of distrust in humans, but out of respect for reality: human error, fragility, and the need for smart supervision.

A Real Example

Think about it: modern electric cars won’t let you accelerate and brake at the same time. The system interprets, corrects, protects.

So how can an aircraft be given full engine power… and then have its fuel supply manually shut off during climb-out, at low altitude?

Toward a New Human-Machine Alliance

We need smart supervision capable of:

  1. Recognizing anomalies
  2. Detecting inconsistencies
  3. Preventing catastrophe
Engineering isn’t just about power or control. It’s also about care, protection, and awareness of limits.
It’s what prevents a single point of failure — human or mechanical — from collapsing the entire system.

Maybe artificial intelligence isn’t enough.
We need an AI sensitive to human fragility, ready to support before it's too late.
Human error, fragility, and smart supervision aren’t just theoretical. They are the basis of responsible, human-centered design.
If you want to learn how to apply these principles in complex mechanical systems, contact GSI Ingranaggi. We’re ready to provide tailored solutions and technical details.
